1. For New Patient appointments: Please complete the following three forms prior to your first appointment. Once you fill out the forms you will provide and receive an email for each to confirm before submitting. This is to ensure privacy. Click each to open: 2. Wear comfortable clothing that you can move in to your appointment. If osteopathic manipulative treatment is indicated after your exam you will need to be able to move easily.

The new patient visit can take about 1.5 hours depending on the complexity of the presenting issues. Follow up visits can take up to 50 minutes.
Insurance Information:
I am an "out-of-network" physician. In order for any or all of your visit and treatment to be covered, your insurance policy must have "out-of network" benefits.
Please check with your insurance carrier prior to making an appointment. I do take Medicare and will submit all claims for Medicare patients.
Unfortunately, I am currently not able to take new Medicare patients.
In most cases the patient is responsible for payment at the time of service. I will submit the claims directly to only some insurance companies and I will do my best to procure proper payment. However, patients are responsible for any balances not paid by insurance.
I will work with people who have financial limitations.
